Don't forget to mention that a time out for patient safety was performed

Local anesthesia

Port site diameters, port types, scope diameter, etc.

Include details of the findings in the body of the dictation, especially areas where there was special attention to the given indication

Try to use correct verbage and grammar

Always end with a statement that the attending was present and scrubbed for the entire procedure (esp clinic cases)
